Summer Camp Registration

I hereby give permission to have staff arrange any emergency medical care, including hospitalization if necessary. In all instances, attempts will be made to contact the guardian first. The participant is responsible for his/her medical coverage.

I hereby release RiverPark Center and all teaching artists from all claims arising from participation in any activity associated with this Day Camp.

I authorize the use of any photos taken during the camp for future non-profit promotional purposes.
